Formly review: a capable form builder with a one-time AI allowance

Formly builds branded forms, surveys and quizzes with logic, live charts, partial responses and Stripe payments. AppSumo sells it from $99 once with unlimited forms and responses, but the AI-generated form count is a lifetime total, and Plan 1 leaves out partial responses.

Bottom line: Formly is a hosted form, survey and quiz builder from a small bootstrapped team in London. It does logic and branching, scores and calculations, live charts that respondents see after they submit, partial responses, Stripe payments and custom domains. On AppSumo it sells five plans, from $99 once for 1 code up to $495 for 5 codes, and every plan has unlimited forms and unlimited responses a month. Two things decide which plan you need. First, the AI-generated forms in each plan (50 on Plan 1) are a lifetime total that never refills, which the founder confirmed on the listing on 22 October 2026. Second, Plan 1 crosses out partial responses, drop-off rates, advanced analytics and duplicate prevention. The listing is refundable up to 60 days and “Not covered by We Got Your Back.”

Best for: marketers, coaches, agencies and small businesses who want Typeform-style forms with logic and payments, and who mostly build forms by hand.

Think twice if: you planned to lean on the AI builder for every form, or you need a payment provider other than Stripe.

Updated 29 October 2026. This Formly review rests on what we read that day: the Formly listing on AppSumo, including all five plans, the deal terms, the newest reviews and questions, and Formly’s own pricing page with its Free, Silver, Gold and Enterprise plans. We haven’t built forms with it, so we describe the plans, the features and what buyers report, not how fast forms load.

What Formly does

Formly runs in the browser. You start from a blank form, a template with logic already built in, or a prompt for its AI builder. The listing’s feature list covers a lot of ground:

  • Building: full-page and conversational layouts, conditional logic and branching, and custom colours, fonts and themes. The listing’s FAQ names question types such as multiple choice, dropdown, image choice, rating and opinion scale.
  • Scoring: a Score Quiz and Calculator feature, which the FAQ says can also be combined with payment questions to run a small online store.
  • Results: live charts for respondents, in bar, donut, radar and gauge styles, so a quiz taker sees their score straight away.
  • Capture: partial responses (from Plan 2), email notifications with PDF responses, hidden fields from URL parameters, reCAPTCHA spam protection and email verification by one-time code.
  • Branding: custom domains, redirect on completion, and removing “Made with Formly.”
  • Reach: more than 25 languages, including right-to-left scripts, and a “GDPR compliant” tick on every plan.
  • Integrations: Google Sheets, webhooks, and “3,000+ integrations via Google Sheets.” The at-a-glance box also names Zapier and Pabbly Connect, and the overview mentions Mailchimp, Slack and HubSpot.

The founder, Urmi Hemani, is listed as co-founder of a company founded in 2020 in the UK, with 1-10 people, described as bootstrapped.

The one thing most buyers miss: AI credits are lifetime

The plan table says “AI-generated forms: 50” for 1 code, 100 for 2, and so on up to 1,000. It doesn’t say per month. Formly’s own site, by contrast, lists “100 AI-generated forms/month” on Silver and “300 AI-generated forms/month” on Gold, so it’s easy to assume the AppSumo figure renews too. That’s the detail most people miss: it doesn’t.

On 22 October 2026 a buyer asked directly. The founder’s answer: “Its total allowance for the lifetime of the account,” with “One-time credits.” When you run out, manual form building stays unlimited, and you can buy extra one-time AI credit add-ons by writing to support. The listing doesn’t publish a price for those add-ons.

Buyers have noticed. A July 2026 review called the AI builder the “real gem” of the product and warned that deleting AI-made forms “doesn’t renew your allocation.” On 14 October 2026 another buyer asked for the listing to say “ONE-TIME ONLY CREDITS” plainly, and reported that a 12-question form “took 20 credits.” The founder replied that one credit is consumed per AI-generated form. We can’t reconcile those two accounts from the pages, so when you test it, write down your credit balance before and after one generation.

Turns out this matters less than it sounds for many buyers. If you’ve got a handful of recurring forms and you’d rather tune them by hand, 50 AI drafts is plenty. If you hoped to spin up a fresh AI form for every client brief, it isn’t.

One-time prices on AppSumo, read 29 October 2026; each plan is a number of stacked codes (1 to 5). Formly's own site lists Free ($0, 3 AI-generated forms), Silver ($19 per user a month or $190 a year, 100 AI-generated forms a month) and Gold ($29 per user a month or $290 a year, 300 AI-generated forms a month), plus Enterprise on request.

Prices checked Oct 29, 2026 on Formly’s own pricing page. Lifetime deals are listings that can end or change, so check the live listing before you buy.

Plan 1 marks four features with a cross: “Prevent duplicate responses,” “Partial responses,” “Advanced form analytics” and “Drop-off rates.” Plans 2 to 5 tick all of them. Everything else on the card is ticked from Plan 1: removing “Made with Formly,” collecting payments, personalised charts, PDF responses by email, hidden fields, reCAPTCHA, email verification, Google Sheets, webhooks, custom domains, redirects, 25+ languages and GDPR. A founder answer on 19 October 2026 adds that the refill link (letting someone resume a half-finished form) is also Tier 2 and up, and that automatic reminders for incomplete forms are “in development” for Tier 2 and up.

AppSumo’s search results print $696 beside the $99 price. That’s the listing’s comparison figure, not a saving we can confirm, and the product page we read showed no struck-through price.

Picking a plan: three worked examples

A coach running one quiz funnel. One quiz, one lead form and a booking form, all on your own domain. Plan 1 covers that: three forms, unlimited responses, custom domain, branding removed, Stripe for paid sessions. Fifty AI drafts is far more than you’ll use. The catch is partial responses. In practice, a long intake quiz is exactly where people drop off, and Plan 1 can’t show you their half-finished answers.

A small marketing team of three. Plan 2 at $198 is the first tier where every feature on the card is ticked, and it adds the 3 users you need. A September 2026 reviewer said their first poll gathered almost 60 full responses and almost 25 partial ones, information they’d otherwise have lost. If that kind of data matters to you, skip the Plan 1 saving.

An agency with fifteen client brands. Plan 2 stops at 10 domains and 10 workspaces, so fifteen clients on their own domains needs Plan 3 at $297, with unlimited domains and workspaces. Now count AI drafts. If you build four new client forms a month with AI, Plan 3’s 200 credits last about four years; at ten a month they last under two. Once they’re gone, you build by hand or buy add-ons.

The deal vs Formly’s own plans

Formly’s pricing page, read on 29 October 2026:

Silver$19 per user a month, or $190 a year100 a month32 GB
Gold$29 per user a month, or $290 a year300 a month104 GB
Enterprise“Get in touch!”Unlimited a monthNot statedNot stated

The deal terms promise “All future Gold Plan updates,” so Gold is the plan to compare. Formly’s Gold lists partial responses, refill link, advanced analytics, drop-off rates, email verification, custom domains (10), your logo and branding removal. AppSumo’s Plan 1 has the domains and branding but not the partial-response or analytics features, so Plan 1 isn’t the same as Gold, whatever the update wording says. Plan 2 lines up much more closely, with more storage and workspaces than Gold lists. Turns out the second code is what buys the Gold-style feature set.

Honestly, the money side is simple. One Gold seat for a year is $290 on Formly’s own page; Plan 2 is $198 once for 3 users. If you’d otherwise pay for Gold for more than a year, the deal costs less, as long as you accept the lifetime AI allowance in place of Gold’s 300 a month. Don’t forget that Formly’s own Free plan exists too: unlimited forms and responses with 3 AI forms, 1 workspace and “Collect payments (10)”, which is a cheap way to try the editor before you buy.

What buyers report

The listing shows 4.62 stars from 239 reviews and 4,155 purchases, and the founder replied to every recent review we read.

Support fixes things. A June 2026 reviewer called it “One of the best deals on AppSumo, full stop” after support “fixed the underlying AI functions” when the AI builder set up logic wrongly. Their one wish was more control over colours, fonts, spacing and mobile rendering.

Finding support can be harder. A July 2026 reviewer rebuilt a client’s market registration form with Stripe payments and praised the receipt it sent. But they found the help articles didn’t solve their pricing calculation, couldn’t find a support email or chat in the app, and got the address from ChatGPT. Once they wrote in, support replied “immediately.” The fix was a missing space before a calculator field.

Partial responses earn their keep. The September 2026 reviewer above described the feature as new and particularly useful.

Questions to watch. On 21 October 2026 a Plan 1 buyer asked for default settings, because theme, custom domain and branding removal had to be set on every new form. The founder’s workaround was to duplicate a template form. On 26 October 2026 a buyer in Venezuela asked about local bank payments and Binance Pay; the answer was that “Formly supports Stripe as the only payment provider,” with redirects and webhooks offered as workarounds.

The terms

The deal terms read:

  • “Lifetime access to Formly – Forms, Surveys & Quizzes (Typeform/Jotform alternative).”
  • “You must redeem your code(s) within 60 days of purchase.”
  • “All future Gold Plan updates.”
  • “Stack up to 5 codes.”

The guarantee line reads “Not covered by We Got Your Back. Listed by the partner, not reviewed by AppSumo.” The listing also says “Refundable up to 60 days.” Like every AppSumo listing, this one can end, so read the plans on the day you buy.

Formly vs Typeform, Jotform and SurveyMonkey

The listing calls Formly an alternative to Typeform, Jotform and SurveyMonkey. We didn’t read or price any of those for this review, so we won’t compare features or costs. The useful question is how you’d use Formly. If you’ve got a subscription form tool mainly for branded forms with logic and Stripe payments, Formly’s feature list covers that. In practice, if you need a payment provider other than Stripe, or an AI builder you can lean on without a ceiling, check the alternative’s own page before switching.

Common mistakes with the Formly deal

Reading the AI count as monthly. It’s a lifetime total. If you’ve got simple forms, skip the AI builder for those and save credits for long ones.

Buying Plan 1 for long forms. Partial responses and drop-off rates start at Plan 2. Long forms are where you’ll want them.

Assuming Plan 1 equals Gold. Many people miss this one. The terms say “All future Gold Plan updates,” but Plan 1 crosses out several Gold features.

Counting on PayPal or local gateways. Stripe only, per the founder’s October 2026 answer.

Forgetting per-form settings. Theme, domain and branding are set per form. Don’t forget to build one template first and duplicate it.

Missing the redemption window. When you stack codes, redeem every one within 60 days of purchase.

Who Formly suits

A coach or consultant with a few quiz funnels. Plan 1, if short forms are enough.

A small marketing team. Plan 2, for 3 users and every listed feature.

An agency with many client domains. Plan 3 or above, for unlimited domains and workspaces.

Anyone needing non-Stripe payments or heavy AI generation. Honestly, skip the deal, or test the limits first.

How to test it inside the refund window

  1. Build your longest real form by hand, with branching and a score or calculation.
  2. Generate one form with AI and check how many credits it used against your count.
  3. Connect Stripe and run a test payment through a payment question.
  4. Point a custom domain at a form and remove “Made with Formly.”
  5. Abandon a form halfway (Plan 2 and up) and confirm the partial response appears in Results.

What we couldn’t check

We didn’t build forms, take payments or measure load times. We couldn’t confirm how many AI credits a single generation uses, since one buyer reported 20 for a 12-question form and the founder says one per form. We also couldn’t find a price for the one-time AI credit add-ons, or confirm which Gold features will reach Plan 1 under “All future Gold Plan updates.” Ask support about all three before day 60.

FAQ: Formly questions

Is Formly a one-time payment on AppSumo?

Yes. The five plans cost $99, $198, $297, $396 and $495 once, read on 29 October 2026, each with unlimited forms and responses.

Do Formly’s AI-generated forms reset every month?

No. The founder confirmed on the listing on 22 October 2026 that the AppSumo allowance is a lifetime total. Extra one-time credits can be bought through support.

Does Formly Plan 1 include partial responses?

No. Plan 1 crosses out partial responses, drop-off rates, advanced analytics and duplicate prevention. Plans 2 to 5 include them.

Which payment providers does Formly support?

Stripe only, according to the founder’s answer on 26 October 2026.

The verdict

Formly is a well-reviewed form, survey and quiz builder with a deep feature list, and the AppSumo deal beats a year of its own Gold plan for most small teams. The decisive limits are clear once you read the questions: AI-generated forms are a fixed lifetime allowance, and Plan 1 leaves out the partial-response tools that make long forms worth running. If you build mostly by hand, Plan 2 is the safe pick; test the AI count and Stripe flow well before day 60.
